Area of Specialization

SPECIALIZED SERVICES

Diabetes Management

Our experienced team provides assistance with all aspects of managing diabetes including: blood glucose monitoring, insulin administration and tracking, exercise plan, menu planning, and education.

We teach our residents living with diabetes the best ways to manage the disease and keep control of their lives. Our registered dietitian will work with you to make smart food choices in the right amounts. Residents enjoy a balanced diet that spreads carbohydrates throughout the day to prevent high blood sugar levels after meals. Our residents are taught how to care for their feet, gums and eyes, and are shown how to incorporate exercise into their diabetes management program.

For residents who require therapy, our physical therapists will develop a customized program to increase your physical activity to improve insulin response and management.

Fall Prevention

As we age, the likelihood of falling increases dramatically. At Clearwater Care Center, our interdisciplinary team is educated on fall risk factors and prevention strategies. Each resident is evaluated upon admission by nursing and occupational and physical therapy and receive an individualized treatment plan if they are risk for falls. We teach residents ways to avoid falls and provide therapy to increase balance. For residents who have a history of falls, we assess the root cause to see why the fall occurred and map an individualized plan of care to prevent recurrence.

Our Fall Prevention Program Includes:

  • Preparation of personalized exercise plans
  • Review and modification of factors leading to previous falls
  • Visual biofeedback to improve walking skills
  • Strength training and therapy for limbs and core muscles

IV Therapy

At Clearwater Care Center, intravenous (IV) therapy may be used to provide nutrition or medication and/ or restore or maintain the body’s normal fluid and electrolyte balance. Our nursing staff administers IV fluids according to physician orders, maintains and monitors injection sites, and changes tubing and dressing regularly.

Trans Parenteral Nutrition (TPN)

Trans Parenteral Nutrition (TPN) is utilized for patients who cannot or should not get their nutrition through eating. TPN may include a combination of sugar and carbohydrates (for energy), proteins (for muscle strength), lipids (fat), and electrolytes. TPN bypasses the gastrointestinal tract by infusing nutrients directly into the bloodstream through an intravenous line.

Your health care provider may measure the effectiveness and side effects of the treatment using laboratory tests and physical examinations. The length of treatment depends on how well the resident responds to the medication.

We administer feeding solutions in accordance with each patient’s nutritional and medical needs.

Respiratory Therapy

Whether treatment is needed for complex respiratory problems, on a short-term basis, or for continuous care, our team has you covered. With treatments for COPD, lung disease, cardiac disease, stroke, neuromuscular disease, and trauma, we are here to help you breathe easier.

Tracheostomy Care

Tracheostomy care is a highly specialized and delicate area of care that many skilled nursing centers are not able to offer. Through ongoing education and training, our staff is committed to providing the highest quality of care for residents who need tracheostomy care. We perform daily tracheostomy and stoma maintenance and comprehensive respiratory services in order to minimize complications, maintain stability and return the individual to their highest level of function.

Wound Care Therapy

Wounds can occur for a variety of reasons, from post-surgery complications to normal skin aging. Our interdisciplinary wound care team includes physical and occupational therapists, registered dietitians and registered nurses. Our wound care team works closely with the resident's physician to prepare a plan of treatment that best addresses individual needs including pain management, healing, and nutritional requirements. As with all treatments, we involve the resident and the family in every step of the treatment plan. Treatment procedures may include nutrition interventions and wound vac treatment.

Specialized supplies, treatments and equipment are utilized to enhance healing and comfort. Our staff is focused on infection-prevention, healing of pressure ulcers and promotion of skin integrity.

We treat a variety of wound conditions and provide an array of services including:

  • Pressure ulcers
